South Central Pa. High Utilizer Collaborative Report Now Available!
Aligning Forces for Quality—South Central PA (AF4Q-SPCA) announces the release of important research and recommendations obtained from a two-year South Central Pennsylvania High Utilizer Collaborative.
The report contains the experience, recommendations and collaboration of five Pennsylvania programs that work with the super-utilizer population in an effort to improve their quality of care while reducing health care spending. The members of the collaborative include WellSpan Health, Lancaster General Health, PinnacleHealth System, Crozer-Keystone Health System and Neighborhood Health Centers of the Lehigh Valley. Funding was provided by AF4Q-SPCA and the Highmark Foundation.
We are honored to be a part of a health care community that chooses to work collaboratively to share experiences and information in order to improve patient care, lower health care costs and improve the overall health or our community. This report can serve as a framework for those working in the field of super-utilizers, for providers who would like to start a super-utilizer program, and as a model to Pennsylvania and the rest of the country as a way to bolster the transformation of the entire health care delivery system.
