High quality health care requires partnerships between providers and patients. Health care teams need to provide high quality care, and patients need to be engaged. Engaging patients can sometimes be a challenge, but it can be done. With time, improved communication, and support, patients can play a greater role in their health. Communication between patients and the health care team is essential for delivering quality care.

Through our Planned Care Collaborative, coaches from the local business community and patient-partners are helping practices become more
efficient, improve staff and provider morale, implement problem-solving
techniques, reduce costs, and improve patient care.
Initiated by the Healthy York Network, the Super Utilzer Pilot was developed. This video showcases the results of the study, and what was done to provide better care for the patient; lessen the cost for the system; and get an understanding of how the health care system contributes to the problem.
